Passport Application - Adult applicants aged 18 and over (one appointment for each application is required)

For an Australian Passport Renewal (PC7), please present the following ORIGINAL documents at your appointment:
1. Completed Passport Application Form available from www.passports.gov.au
2. Two (2) recent photographs, your full name written in block letters on the reverse in black pen
3. Previous Australian passport

For an OVERSEAS Passport Application (PC8), please present the following ORIGINAL documents at your appointment:
1. Completed Passport Application Form available from www.passports.gov.au
2. Two (2) recent photographs, one endorsed on the reverse side by the guarantor by writing 'This is a true photo of (applicant's full name)' and signing in black pen
3. Previous Australian passport (if applicable)
4. Local birth certificate or Australian birth certificate issued by the Registry of Births Deaths and Marriages
5. Australian Citizenship Certificate
6. Evidence of name change if applicable
7. Photo ID e.g. foreign ID or driver's license
8. Additional proof of identification e.g. bank card, health card etc
9. Proof of Address e.g. utility bill with name and address

Witnessing the signing of an Australian form or document (one appointment for each service is required)

- Your completed UNSIGNED document.
- Proof of identity (passport, local ID).

- Statutory Declaration for some States or Territories
